Why is there a line below the top side and then again the third placed side?

4th play 7th and 5th play 6th in Quarter Finals. The two winners play 2nd and 3rd in Semis and then a normal final!

Also highest placed teams are home.Only the one game,so the highest finishing teams have a advantage.

Peanuts compared to the EFL.The BT televised deal is not worth much.I can’t remember the number when it was discussed on the Price of Football podcast a while back,but it was nothing like the TV money for a L2 game.
It’s a very cheap schedule filler for BT Sport.

I expect the cost of televising the game outweighs what the clubs receive.
