Well i agree even though you are not sure. It would not be 'a bit of a go ' from the ref but decisions by him leading to penalties so why wouldn't the managers change their instructions accordingly ?

I don't know about the last 15 minutes but certainly bringing on substitutes with just a few minutes to go interrupts any momentum a side has gathered and diminishes the game as a spectacle.
I agree it is annoying when play is stopped in this way when a team is in a good position and the initiative is lost .As you say it's convention not anything the referee can insist upon. If the referee is unable to recognise the cheating it doesn't seem anything can be changed unfortunately.
Same rule but if play is stopped with one side in clear possession the practice nowadays is for an uncontested drop ball where the other side returns the ball to the ones holding the ball at the stoppage.

Clearly he was but unfortunately this happens frequently and is ignored by the officials. No reason why the 4th official should not be charged to watch out specifically for this type of infringement - also encroachment into the area before the penalty is taken.
